Central Providence Healthy Aging Network (CP-HAN)

The Central Providence Healthy Aging Network is a “village” membership club that connects older adults with community resources. Our goal is to help older adults maintain their independence and quality of life.

The CP-HAN connects older adults with an array of opportunities and supports, including daily meals, exercise, enrichment, transportation, minor home repair, home visiting, nutrition education, access to food support, and healthcare advocacy. CP-HAN utilizes staff, volunteers, and community partnerships to keep seniors active and healthy. CP-HAN is a partnership with ONE|Neighborhood Builders and part of the Central Providence Health Equity Zone initiative. For more information, contact Michele Matott at (401) 421-4722.
